Preventative Health & Chronic Condition Support
What is it?
Preventative health consists of general practice measures taken for chronic condition prevention with screening, immunisation and patient centred care planning to optimise your current health status. Chronic conditions include cardiovascular disease, diabetes, lung conditions such as asthma, COPD, emphysema, obesity, hypertension, chronic kidney disease, chronic infectious diseases, chronic fatigue, chronic joint conditions.
Chronic conditions:

have complex and multiple causes

may occur as a single condition in a person, or along with other diseases (comorbidity)

usually progress gradually

can occur at any age, although they are more common as people get older

can affect quality of life

can create limitations and disability

are not usually immediately life-threatening, but can shorten life expectancy
GP management plans and better treatments allow people to live longer with chronic conditions.

GP Management Plans for chronic disease with options for referral to Allied Health services, such as Physiotherapist, Exercise Physiologist, Dietician, Podiatrist, Diabetes Educator.

Who does it?
All of our GPs provide individualised patient centred chronic disease health advice for all ages.
Our nurses provide additional patient support with education, resources, collaborative care plans, immunisations and pathology services, all incorporated within the GP consultation; please advise our staff if you wish to see our nurse at the time of the booking.
A GP Management Plan can provide Medicare rebates for up to 5 visits a year for eligible allied health support providers such as Physiotherapist, Exercise Physiologist, Dietitian, Diabetes Educator, Chiropractor, Psychologist, Speech Pathology, & Occupational Therapist (out of pocket costs may apply with providers).
